Regimen : Stanford V for Hodgkin lymphoma.
Cycle: three 4-week cycles. Total regimen is 12 weeks.

• Doxorubicin, 25 mg/m2, IV, weeks 1, 3, 5, 7, 9 and 11
• Mechlorethamine, 6 mg/m2, IV, weeks 1, 5 and 9
• Vinblastine, 6 mg/m2, IV, weeks 1, 3, 5, 7, 9 and 11
• Vincristine, 1.75 mg/m2, IV, weeks 2, 4, 6, 8, 10 and 12.
• Bleomycin, 5 units/m2 , IV, weeks 2, 4, 6, 8, 10 and 12.
• Etoposide, 60 mg/m2 per day IV on 2 consecutive days in weeks 3, 7, 11
• Prednisone, 40 mg/m2 PO every other day for 10 weeks then tapered by 10 mg PO every other day between week 10 and 12
For prednisone calculate the total PO dose in 12 weeks for a patient measuring 5 feet 10 inches in height and weighing 341 lb.

Click on the button below to see the answer and explanations

lb equals 4382.00 mg kg

• The patient height
= 5 feet 10 inches
= ( 5 × 12 + 10 ) inch
= 70 inch
= 70 × 2.54 cm
= 177.8 cm

• patient weight
= 341 lb
= 341 × 0.454 kg
= 154.8 kg

• BSA for the patient is

BSA = \quad sqrt(( Height (cm) × Weight (Kg) )/(3600))

 \quad \quad \quad = sqrt(( 177.8 \quad cm × 154.8 \quad Kg )/(3600)) = 2.7650 m2

• ### Here for prednisone :

patient dose for every other day
= ( 40 \quad mg)/(1quad m^2) × 2.7650 m^2
= 111 mg

Hence, dose for first 10 weeks
= 111 \quad mg × 35 (days of treatment)
= 3885 mg

And dose for 11^(th) and 12^(th) weeks (7 days of treatment tapered by 10 mg)
= ( 101 + 91 + 81 + 71 + 61 + 51 + 41 ) mg
= 497 mg

Therefore, total dose for 12 weeks cycle
= 3885 mg + 497 mg = 4382 mg Ans.
