Regimen : Stanford V for Hodgkin lymphoma.
Cycle: three 4-week cycles. Total regimen is 12 weeks.

  • Doxorubicin, 25 mg/m2, IV, weeks 1, 3, 5, 7, 9 and 11
  • Mechlorethamine, 6 mg/m2, IV, weeks 1, 5 and 9
  • Vinblastine, 6 mg/m2, IV, weeks 1, 3, 5, 7, 9 and 11
  • Vincristine, 1.61 mg/m2, IV, weeks 2, 4, 6, 8, 10 and 12.
  • Bleomycin, 5 units/m2 , IV, weeks 2, 4, 6, 8, 10 and 12.
  • Etoposide, 60 mg/m2 per day IV on 2 consecutive days in weeks 3, 7, 11
  • Prednisone, 40 mg/m2 PO every other day for 10 weeks then tapered by 10 mg PO every other day between week 10 and 12
For prednisone calculate the total PO dose in 12 weeks for a patient measuring 6 feet 2 inches in height and weighing 389 lb.

lb equals 4802.00 mg kg

  • The patient height
    = 6 feet 2 inches
    = ( 6 × 12 + 2 ) inch
    = 74 inch
    = 74 × 2.54 cm
    = 187.96 cm

  • patient weight
    = 389 lb
    = 389 × 0.454 kg
    = 176.6 kg

  • BSA for the patient is

BSA `= \quad sqrt(( Height (cm) × Weight (Kg) )/(3600))`

    ` \quad \quad \quad = sqrt(( 187.96 \quad cm × 176.6 \quad Kg )/(3600)) = 3.0365 ` m2

  • Here for prednisone :

patient dose for every other day
    = `( 40 \quad mg)/(1quad m^2) × 3.0365 m^2
    = 121 ` mg

Hence, dose for first 10 weeks
    = ` 121 \quad mg × 35` (days of treatment)
    = 4235 mg

And dose for `11^(th)` and `12^(th)` weeks (7 days of treatment tapered by 10 mg)
    = ( 111 + 101 + 91 + 81 + 71 + 61 + 51 ) mg
    = 567 mg

Therefore, total dose for 12 weeks cycle
    = 4235 mg + 567 mg = 4802 mg `Ans.`
